Abstract

A second-degree birth canal tear is a tear in the perineal tissue that involvesdeeper tissues such as muscle and connective tissue. These tears can occur duringor after childbirth, and these tears can be very painful. According to World HealthOrganization (WHO) data, in 2019 the number of Maternal Mortality Rate (MMR)was 228/100,000 Live Births (KH) and increased by 359/100,000 KH. This figureis still high when compared to the Sustainable Development Goals (SDG'S) targetin 2019 MMR of 306/100,000 KH (WHO, 2019). In 2020, it is known that inIndonesia the incidence of perineal rupture in maternity mothers was experienced by 83% of mothers giving birth vaginally, found from a total of 3,791 mothers whogave birth spontaneously vaginally, 63% of mothers received perineal stitches,namely 42% due to episiotomy and 38% due to spontaneous tearing (Ministry of
Health RI, 2021). According to the North Sumatra Provincial Health Office,precisely in the city of Medan (2020), reported 202 maternal deaths, which weredivided into 53 maternal deaths, 62 maternal deaths, and 62 maternal deaths. Theage group responsible for many maternal deaths is the 20-34 year age group. 75%of women who give birth vaginally experience perineal rupture. Of the total 1951 natural vaginal deliveries, 57% of mothers experienced perineal closure (28% dueto episiotomy and 29% due to natural tearing). 19% had perineal rupture, 11% old partus, 10% bleeding each and eclampsia (Ministry of Health, 2020). From theresults of a study conducted by the author entitled Post Partum SpontaneousMaternity Nursing Care in Mrs.N with Acute Pain of Grade II Perineal Rupture atthe Santi Meliala Medan Inpatient Primary Clinic in 2023, data was obtained that
27 out of 35 mothers gave birth to perineal rupture. Therefore, the author isinterested in raising the case of perineal rupture as a case study to compile and provide Maternity Nursing Care for mothers who experience perineal rupture. Theresearch method used is a type of case study that aims to explore directly the problems experienced by clients with Perineal Rupture Acute Pain Diagnosis at
Santi Meliala Medan Inpatient Primary Clinic, conclusions were obtained, namely:After conducting Spontaneous Post Partum Maternity Nursing Care on Mrs.N with Acute Pain of Grade II Perineal Rupture at Santi Meliala Inpatient Clinic in 2023 for four days using 5 nursing processes, namely: Assessment, Nursing Diagnosis,Nursing Intervention, Nursing Implementation, and Nursing Evaluation obtained results: Mrs. N was able to apply deep breath relaxation techniques to reduce pain arising from birth canal trauma, Mrs. N was able to apply how to do hygiene in the vulvar area, Mrs. N was able to overcome pain that appeared when doing mobility.
