Abstract

ntroduction: Perineal rupture is a spontaneous tear in the birth canal, perineal rupture is also the second highest maternal mortality rate (MMR) in Indonesia. Perineal rupture is the occurrence of injury (tear) in the perineal muscles during the second stage of labor and can recur in subsequent deliveries. Objective: This study is able to provide comprehensive midwifery care to Mrs. M starting from the
third trimester of pregnancy to the Family Planning program at the Niar Primary Clinic, Medan Amplas District with a midwifery care management approach in accordance with the seven steps of Helen Varney and SOAP. Method: The research care provided to Mrs. M, 24 years old, G2P3A0, Gestation 39 weeks 4 days is continuous, continuity of care (COC) the author uses a descriptive research method, namely a case study. Data collection was carried out by interviews, observations and physical examinations of pregnancy, childbirth, postpartum, newborns, and family planning in Mrs. M with perineal rupture. Results: checking the completeness of the placenta and the presence of second
degree birth canal tears (from the vaginal mucosa, posterior commissura, perineal skin to the perineal muscles) and the placenta was born complete with an umbilical cord length of 50 cm, 19 contydons, and a diameter of 20 cm. Conclusion: after Helen Varney's steps were taken in Mrs. M's case by carrying out hecting treatment, it was more effective for the mother giving birth.
