Abstract

Hypertension is a condition that often present without symptoms, in which abnormally high blood pressure within the arteries increases the risk of various diseases. As people age, systolic pressure tends to rise until about the age of 80, while diastolic pressure continues to increase until around the age of 55-60, after which it begins to decrease gradually or even drastically. Objective: The general
objective is to develop and implement geriatric nursing care management for patients with hypertension at the UPTD Social Services Center for the elderly in Binjai in 2024. Method: This study uses a case study design with a descriptive qualitative approach. Subject: The research subject is one patient diagnosed with Hypertension. Data collection was carried out through various method, including interviews, direct observation of the patient’s condition, and physical examination to obtain a more accurate picture of the patient’s status. Results: Based on the assessment conducted, three nursing diagnoses were identified: Acute Pain, Disturbed Sleep Pattern, and Decreasesed Cardiac Output. Intervension included identifying the pain scale,
assessing activity and sleep patterns, and monitoring blood pressure. Evaluation showed significant improvement in the patient’s physical and psychological parameters. Conclusion: Health education provided can improve the success of
delivering geriatric nursing care for patients with hypertension.
