Abstract

Ischemic stroke is the most common type of stroke, occurring due to a blockage in the cerebral blood vessels that causes a reduction in blood flow and oxygen supply to brain tissue. This condition leads to rapid brain cell death and can result in neurological dysfunctions such as paralysis, speech disorders, and loss of consciousness. Objective The general objective is to develop and implement Medical-Surgical Nursing Care Management for patients with ischemic stroke at H. Adam Malik General Hospital, Medan, in 2025. Method This study uses a case study design with a descriptive qualitative approach. Subject The subject of this research is a patient diagnosed with ischemic stroke. Data collection was conducted through various methods, including interviews, direct observation of the patient’s
condition, and physical examinations. Results Based on the assessment results, three nursing diagnoses were identified: Decreased intracranial adaptive capacity, Impaired physical mobility, and Impaired swallowing.The interventions
implemented included monitoring signs and symptoms of increased intracranial pressure, teaching simple mobilization activities such as sitting on the bed, and
monitoring swallowing ability. Evaluation showed significant improvement in the patient’s physical and psychological parameters.Conclusion: Health education provided during care improved the success of delivering medical-surgical nursing
care for patients with ischemic stroke.
