According to (WHO) 2020, the maternal mortality ratio (MMR) is 295,000 deaths with the causes of maternal deaths being high blood pressure during pregnancy (preeclampsia and eclampsia), bleeding, postpartum infection and unsafe abortion. Perineal rupture is a tear that occurs in the birth canal spontaneously where the factors that influence
the occurrence of perineal rupture are primigravida mothers who give birth to babies> 3500 grams, head circumference> 35 cm, shoulder dystocia factors, the position of the mother
when receiving, and deliberate episiotomy. And for handling in degrees I and II, namely by hectoring so that it can be reunited. The purpose of the research was to carry out midwifery care for childbirth on Mrs. E with first degree perineal rupture at the Pratama Niar Clinic, Medan Amplas District, Medan City, North Sumatra Province in 2024 comprehensively. The research method used was descriptive using 7 helen varney steps carried out on November 22, 2023. The researcher has carried out ongoing care with the results of the delivery mother safely and the baby was born healthy, the postpartum period runs normally and has used natural contraception with the MAL method.
