Manajemen Asuhan Keperawatan Gerontik yang Service Excellent dengan Hipertensi pada Ny. Y di UPTD Pelayanan Sosial Lanjut Usia Binjai Dinas Sosial Provinsi Sumatera Utara Tahun 2025

Hypertension is a medical condition characterized by increased blood pressure, with systolic pressure ≥140 mmHg and diastolic ≥90 mmHg based on repeated measurements. The purpose of this study is to plan and carry out Gerontic Nursing Care by conducting assessments, formulating and determining diagnoses, formulating interventions, implementing implementation and conducting nursing
evaluations. Method used is a case study type that aims to determine problems with Hypertension in the UPTD for Elderly Social Services in Binjai starting from March 24-29 and April 8-9, 2025. Subject of this study was Mrs. Y who had
hypertension. Results of the study showed that Mrs. Y complained of pain in the nape of the neck with a pain scale of 7, experienced sleep disturbances, complained of sleep dissatisfaction and functional status to perform daily activities with a level of dependence on others. Therefore, the diagnosis raised was Chronic Pain related to emotional stress, Sleep disturbances related to lack of sleep control, Activity intolerance related to Weakness and planned interventions include monitoring the pain scale to decrease using the PQRST technique, monitoring vital signs,performing deep breathing relaxation techniques, massaging the painful area, performing hypertension exercises and providing health education. Conclusion: The nursing care provided in treating the patient showed an increase in the ability to implement the correct treatment program and management of hypertension in the elderly.
