Abstract
Chronic Obstructive Pulmonary Disease (COPD) is aprogressive respiratory disorder characterized by airflow obstruction and increased mucus production, which can decrease the patient’s quality of life. This study aims to objective the
management of medical-surgical nursing care using a service excellence approach in COPD patients so that the care provided can enhance comfort, satisfaction, and clinical outcomes. This method used a descriptive design. The subject of this case study was one patient, Mr. H, who was treated in Room RA6 at the Ministry of Health Adam Malik Hospital, Medan, in 2025. Data were collected through interviews, direct observation, physical examination, and medical record review. The nursing care process was carried out according to the five stages of the nursing process, including assessment, diagnosis, planning, implementation, and
evaluation. The results showed that Mr. H experienced shortness of breath, productive cough, and fatigue due to impaired gas exchange and ineffective airway clearance. The interventions provided included pursed-lip breathing exercises, oxygen therapy, semi-Fowler’s positioning, family education, and collaboration with the medical team in administering bronchodilators. The evaluation showed an
improvement in oxygen saturation, a decrease in thefrequency of dyspnea, and an increase in the patient’s ability to perform light activities. In conclusion, the application of service excellence–based nursing care in COPD patients can
improve therapy effectiveness, accelerate recovery, and increase patient and family satisfaction with nursing services.
