Abstract
The World Health Organization (WHO) estimates that 800 women die every day due to complications of pregnancy and childbirth, about 99% of all maternal deaths occur in developing countries, about 80% of maternal deaths are due to increased complications during pregnancy, childbirth and after delivery (WHO, 2010). 2019) Through the implementation of continuity of care, namely by checking and monitoring mothers during pregnancy, childbirth, postpartum, newborns and family planning, it can at least minimize MMR and IMR. The implementation of this midwifery care has been handed over to Mrs. F at the sondang Clinic 2024 This care shows that the pregnancy in Mrs. S went well, the mother was 38 weeks pregnant when she approached delivery. The mother's delivery went well, the newborn looked fit, the mother's condition was good. The postpartum period is carried out for six weeks normally, the mother uses the KB implant as a means of contraception. Researchers suggest that health workers, especially in the field of implementing continuity of care in the field and in the community, can help reduce
maternal and infant mortality rates in Indonesia.
