Abstract

Gout arthritis is a degenerative disease of the joints that is commonly experienced by the elderly, characterized by the accumulation of uric acid crystals in the joints, causing inflammation and pain. This case study was conducted at the
UPTD Elderly Social Service Center in Binjai, North Sumatra, focusing on the implementation of gerontic nursing care management based on the principle of service excellence for Mrs. R, who was diagnosed with gout arthritis. The purpose
of this case study is to provide nursing care with the service excellence approach to the elderly patient, Mrs. R, with gout arthritis at the UPTD Elderly Social Service
Center in Binjai, North Sumatra, in 2025. The research Method used was descriptive with a case study approach. Data were collected through interviews, observations, physical examinations, and documentation. The nursing care
interventions included health education, foot massage to reduce pain, range of motion (ROM) exercises to improve physical activity, and relaxation techniques such as deep breathing exercises. The Results showed that the application of gerontic nursing care with the service excellence principle for Mrs. R with gout arthritis was able to improve the quality of life of the elderly through health education, foot massage, and deep breathing techniques. Conclusion The findings also indicated that the implementation of excellent nursing care plays an important role in optimizing the physical and psychological condition of the elderly and
supports the achievement of SDGs goal 3.4 regarding the reduction of premature mortality due to non-communicable diseases.
