Mental disorders are conditions that affect a person's feelings, thoughts, behavior, and social interactions. In Indonesia in 2023, 90% of the 491,985 people with mental disorders had received health services. One common symptom is social isolation, where individuals avoid direct interaction because there is no space to share thoughts and feelings. They tend to be alone, pay less attention to their surroundings, and have difficulty communicating. Withdrawal is a typical behavior in patients with mental disorders, characterized by decreased self-confidence and distancing from the surrounding environment. The purpose of this study was to be able to implement Mental Health Nursing Care Management with Social Isolation Withdrawal at Prof. Dr. Muhammad Ildrem Mental Hospital, North Sumatra Province in 2025. This research method is descriptive with a nursing care approach used, including assessment, diagnosis, planning, implementation, and evaluation. The research subject used was one patient who experienced mental disorders with social isolation withdrawal. The results of the study after four days of intervention, the patient showed positive developments such as the ability to make
acquaintances, start conversations, make eye contact, and interact in groups. Conclusion: The nursing care carried out in treating patients showed an increase in the patient's social abilities. Nurses continue interventions according to a daily schedule to maintain and improve outcomes throughout the course of care.
