Asuhan Keperawatan Jiwa Pada Tn.S Dengan Isolasi Sosial Di Ruangan Gunung Sitoli Rumah Sakit Jiwa Prof. Dr. M Ildrem Kota Medan Tahun 2025

Social isolation is one of the disturbances in social functioning often experienced by patients with schizophrenia, characterized by withdrawal, communication difficulties, and a decreased interest in interacting with others. This condition requires the implementation of psychiatric nursing care management that is professional and oriented toward service excellent to restore the patient’s social functioning. Objective The general objective of this study was to implement psychiatric nursing care management with service excellence for Mr. S, who experienced social isolation at Prof. Dr. Muhammad Ildrem Mental Hospital, North Sumatra Province, in 2025. Methods This study used a case study design with a descriptive approach. Subject The research involved one patient diagnosed with social isolation. Data were collected through interviews, direct
observation, and nursing documentation review. The nursing management process consisted of planning, organizing, implementing, and evaluating stages based on the psychiatric nursing care standards SP 1–SP 3. Result Based on the
assessment, one primary nursing diagnosis was identified social isolation. Interventions included establishing a trusting relationship, helping the patient recognize the causes of social isolation, and encouraging social participation. Conclusion The application of psychiatric nursing care with a service excellent approach and therapeutic communication can improve nurses success in
providing quality mental health nursing care for patients experiencing social isolation.
