Abstract
Background: Continuity of care (CoC) is the concept of providing integrated and continuous health services from pre-pregnancy, pregnancy, childbirth, postpartum, newborn care, and family planning (FP) services. Gestational hypertension is a condition of high blood pressure (≥140/90 mmHg) that appears after 20 weeks of gestation without proteinuria and generally improves after delivery. The purpose of this study was to provide comprehensive continuous midwifery care to pregnant women with gestational hypertension, childbirth, postpartum, newborns, and family
planning (FP) for Mrs. S at the Sumiariani PMB, Medan Johor District, Medan City in 2025. Method: The research method used was a descriptive method, which was studied to produce an objective description of the situation. This descriptive study also utilizes a method by which researchers attempt to describe the objects and subjects studied in depth, broadly, and in detail, including pregnant women experiencing
gestational hypertension, labor, postpartum, newborns, and family planning at the Sumiariani Prenatal Care Center (PMB) in Medan. The population in this study was all pregnant women with gestational hypertension stage II who visited the
Sumiariani Prenatal Care Center in Medan. The subject in this study was one second-trimester pregnant woman with gestational hypertension who visited the Sumiariani Prenatal Care Center, Mrs. S, 26 years old, G1P0A0, who had
gestational hypertension, labor, postpartum, newborns, and family planning. Therefore, midwifery management was required. Results: The researchers provided continuous midwifery care for Mrs. S, starting from the pregnancy with gestational hypertension, labor, postpartum, newborns,
and family planning. The conclusion: after conducting routine continuous midwifery care management for pregnant women with Gestational Hypertension, childbirth, postpartum,
newborn, and family planning was that the mothers experiencing Gestational Hypertension had been resolved with the care provided by the researcher.
