According to data from the WHO (World Health Organization), maternal mortality rates remain very high, with an average of 295,000 women dying during pregnancy and childbirth in 2018. Among Asian countries, Laus had the highest maternal mortality rate at 197 per 100,000, and the infant mortality rate (IMR) was 20 per 1,000 live births. In comparison, Indonesia in 2021 had an AKI of 305 per 100,000 live births, and in North Sumatra in 2021, the AKI reached 119 cases and the IMR reached 299 cases (( Continuity of Care (COC) is an effort to help monitor and identify possible complications accompanying mothers and babies from pregnancy to the use of contraception by mothers. Objective: To provide midwifery services in the form of Continuity of Care (COC) starting
from pregnancy, childbirth, the postpartum period, newborn care, and family planning. Method: The method used for comprehensive care for Mother A was descriptive research, which systematically describes phenomena based on the type used, namely a case study. Results: Examination of the
completeness of the placenta and the presence of a second-degree tear of the birth canal (from the posterior commissure of the vaginal mucosa to the perineal skin to the perineal muscle) and a complete placenta with a cord length of 50 cm, 19 cotiledons, and a diameter of 20 cm.Conclusion:
Helps to understand the process of pregnancy to family planning. In Mother A, when the third trimester of pregnancy began in November 2024 until March 2025, the author provided COC guidance with the aim of successful Continuity of Care.
