Abstract
BACKGROUND : Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) are the main indicators of a country's health status. Continuity of Care (COC) services are ongoing health monitoring efforts to minimize complications
during pregnancy through family planning.
OBJECTIVE: To provide comprehensive midwifery care for Mrs. N starting from the third trimester of pregnancy, childbirth, postpartum period, newborn care, to family planning services.
METHODOLOGY: This study used a descriptive design with a case study approach conducted on Mrs. N, aged 19 years, at Klinik Pratama Salbiyana.
RESULTS: Antenatal care was carried out in six visits according to standards. The delivery occurred normally and spontaneously at 38–40 weeks of gestation. Postpartum care was conducted in four visits, and newborn care in three visits, all within normal limits without complications.
CONCLUSION: The implementation of Continuity of Care (COC) for Mrs. N was carried out safely and effectively. Although there was a maternal age risk factor (19 years), no complications were found during the process.
