Abstract
Background: Type II diabetes mellitus is a chronic disease characterized by elevated blood glucose levels due to insulin resistance, which can lead to diabetic ulcers, resulting in delayed wound healing and the need for continuous nursing care. Objective: To implement nursing care management for Mr. D with type II diabetes mellitus and diabetic ulcer at RA 4 Ward, Ministry of Health RSUP H.
Adam Malik, in 2025. Methods: This study used a descriptive method that systematically described the nursing process, including assessment, diagnosis, intervention, implementation, and evaluation, using an assessment form instrument. The study was conducted at the Ministry of Health RSUP H. Adam Malik from April 26–28, 2025. Subject: The subject of this study focused on one patient, Mr. D, with type II diabetes mellitus and diabetic ulcer. Results: The three-day assessment identified five nursing diagnoses: ineffective peripheral tissue perfusion, unstable blood glucose levels, impaired skin integrity, activity intolerance, and risk for infection. Interventions were implemented according to the care plan, including circulation care, hyperglycemia management to stabilize glucose levels, simple wound care resulting in a drier wound condition, range of motion (ROM) exercises, early mobilization, and infection prevention through proper education and care. The interventions reduced pain, improved circulation, achieved better glucose control, enhanced wound healing, increased mobility, and improved Mr. D’s quality of life. Conclusion: The case of type II diabetes mellitus was selected as the focus due to the significant contribution of excellent nursing care in managing type II diabetes mellitus with diabetic ulcer, supporting both the physical and
psychological recovery of Mr. D.
