Background Continuity of care is midwifery care that is carried out continuously or continuously from the beginning of pregnancy, birth, newborns, postpartum, family planning. Maternal mortality rate (MMR) Based on the 2020 census of 189 / 100,000 live births, almost reaching the 2024 RPJMN target of 183 / 100,000 live births. Health efforts to reduce MMR are carried out during pregnancy, childbirth,
and postpartum. Coverage of ANC K6 visits, childbirth in health facilities, and postpartum were 74.4%, 87.2%, and 85.7% respectively in 2023 (Ministry of Health 2023). The purpose of this study was to carry out comprehensive continuous midwifery care for pregnant women with Gestational Hypertension, childbirth, postpartum, BBL, and KB in Mrs. Y at the Fajar Clinic, Tanjung Morawa District.
Research Method The type of research used for comprehensive care for Mrs. Y is descriptive observational research, which systematically describes a phenomenon with what the individual is. In this case, the author wants to describe midwifery care for Mrs. Y. Subject The subject used was a pregnant woman Mrs. Y aged 27 years, G2P1A0, given care starting from pregnancy, childbirth, newborn, postpartum to family planning. The results of the researcher have carried out continuous midwifery care management for Mrs. Y starting from pregnancy with Gestational Hypertension, childbirth, postpartum, newborn, family planning. Conclusion after carrying out routine continuous midwifery care management for pregnant women with Gestational Hypertension, childbirth, postpartum, newborn, family planning, it was concluded that mothers who experience Gestational
Hypertension have been resolved with the care provided by the researcher and there are several gaps between existing theory and practice.
