Abstract
Continuity Of Care (COC) care, namely providing continuous midwifery services given to women, providing individual care, and providing continuous care. Continuous midwifery care can provide information and service benefits for families and pregnant women and can benefit women in making decisions regarding individual health (Raraningrum, 2021). The purpose of this study was to determine Midwifery
Continuity Of Care (COC) Care for Pregnant Women with Severe Anemia, Maternity, Postpartum, Newborn and Family Planning for Ny. A In independent midwife practice Diana Eva N,S.Keb, Medan Perjuangan district in 2021. Method: The type of writing for this case study is descriptive writing. To describe the health of individuals used descriptive writing. In this case, the author tries to describe the care provided by midwives to pregnant women. Results: Mrs. A, 17 years old G1P0A0 with HPHT June 6 2021, complaints on TM1 nausea vomiting, TM2 dizziness and fatigue. At the antenatal visit, 4 examinations were carried out at the midwife and 2 times at the doctor's practice, it was found that at TM 3, mothers often experience dizziness on HB examinations, they get 7% hemoglobin, in this examination, mothers are advised to
carry out deliveries at a more complete health facility, namely the home hospital to avoid complications during labour. Delivery was carried out in the hospital with
indications of low hemoglobin. Normal babies born midwives take care of newborns to handle normal baby care. The postpartum period has no complications and runs normally, after the postpartum period the mother chooses a 3-month injectable contraceptive because it does not interfere with milk production.
