Low self-esteem is a feeling of worthlessness, meaninglessness and prolonged low self-esteem due to negative evaluation of oneself or one's abilities. If this case is not addressed, more serious problems can occur, such as social isolation, hallucinations, and a high risk of violent behavior. Purpose of this study is to implement excellent service-based psychiatric nursing care management for Mr. M with low self-esteem nursing problems in the work area of Prof. Dr. Muhammad Ildrem Mental Hospital Medan in 2025. The research method is a descriptive approach with a nursing care approach used including Assessment, Diagnosis,
Intervention, Implementation, and Evaluation. The research subject used was a patient who suffered from mental disorders with low self-esteem. Data collection was carried out through various methods, namely interviews, direct observation of the patient's condition, and physical examination to obtain a more accurate picture of the patient's condition. The results of this study After the assessment, three nursing problems were found in Mr. M, namely low self-esteem, social isolation and verbal communication disorders. Conclusion After being given nursing care for 4 visits with a 30-minute interval, the client's condition improved. The client was able to build good relationships with clients, the client was able to identify positive aspects that he had, the client was able to assess the abilities that could still be used, the client was able to choose and determine activities to be trained,
the client was able to train the selected positive aspects and was able to make a daily activity schedule and do it.
