Abstract

Background : Continuity of Care (COC) represents an essential framework in maternal and neonatal health services, underscoring the sustained involvement of midwives throughout the reproductive cycle, including pregnancy, childbirth, postpartum, newborn care, and family planning. Amniotic fluid serves a critical protective function during pregnancy; however, meconium-stained amniotic fluid (MSAF), characterized by the discoloration of amniotic fluid due to meconium, may increase the risk of adverse maternal and neonatal outcomes. Purpose : of this study aimed to provide comprehensive midwifery care through the COC approach for cases involving pregnancy, childbirth with MSAF, postpartum,
newborns with moderate asphyxia, and family planning. The care process was documented using Varney’s Seven-Step framework and SOAP. Methode : Of This Reaserch Is descriptive research design was employed to objectively present and analyze the clinical situation in detail.
Result : The subject of the study was Mrs.D, a 36-year-old multiparous woman (P3A0), who met the inclusion criteria and consented to receive continuity of care at BPM Putri Maysaroh.Findings indicated that the participant actively engaged in all stages of care. The implementation of continuous and comprehensive midwifery management demonstrated effective resolution of complications,
particularly childbirth with MSAF and neonatal moderate asphyxia. Conclusion : the application of continuity of care yielded positive outcomes in maternal and neonatal health, confirming its value as a holistic and systematic approach within midwifery practice.
